Position Description
Pride Health is hiring a Home Health RN to support our client’s medical facility in Denver, CO. This is a 13-week assignment (with the Possibility of extension or conversion) and a great opportunity to start working with a top-tier healthcare organization!
Registered Nurse (Home Health/Hospice)
Location: Denver, Colorado, 80221
Shift: 8 hours Guaranteed Hours: 40/week
Contract Length: 13 Weeks with possible extension.
The Registered Nurse (RN) delivers skilled home health and hospice nursing care to patients of all ages in their place of residence. This role coordinates care with the interdisciplinary team, patients/families, and referring agencies, ensuring high-quality, continuous, and patient-centered services.
Key Responsibilities
- Assess home health/hospice patients in routine and emergent situations, identifying physical, psychosocial, and environmental needs.
- Develop, implement, and update individualized Plans of Care (POC).
- Document all visits, assessments, interventions, and changes in patient status accurately and timely.
- Provide patient and family education on disease management, medications, safety, and self-care.
- Utilize proper infection control practices and ensure safe use of medical equipment.
- Manage pain assessment and appropriate pain interventions.
- Supervise HHAs/STNAs/LVNs to ensure quality and continuity of care.
- Arrange and coordinate referrals to outside services or agencies as needed.
- Monitor compliance with third-party payer requirements.
- Participate in on-call rotation and interdisciplinary team meetings.
- Communicate effectively with patients, families, providers, and internal team members.
Skills & Qualifications
- Current state RN license (required)
- 1–2 years clinical RN experience; 2+ years home health experience required
- Homecare Homebase (HCHB) and OASIS experience required
- CHPN certification preferred
- Current CPR certification, valid driver’s license, reliable transportation, and insurance
- Competency in wound care, venipuncture, IV therapy, phlebotomy, catheter/drain care, inhalation therapy, and home medical equipment use
- Strong communication, organizational, and critical-thinking skills
- Ability to work independently, manage time effectively, and travel locally
- Maintains required licensure and continuing education
Education
- Associate Degree in Nursing or Nursing Diploma required
- Bachelor of Science in Nursing preferred.
